Your Personal Information and Emergency Room Card

In response to many patient and colleague requests, ALS Worldwide has created an informational ID and emergency room card that can assist those with ALS when driving a car or in public places, such as a restaurant or theater. When entering a hospital or other medical facility becomes necessary, medical practitioners will have immediate and accurate knowledge about your condition, medications, emergency contact and other related information.
