Your Personal Information and Emergency Room Card
In response to many patient and colleague requests, ALS Worldwide has created an informational ID and emergency room card that can assist those with ALS when driving a car or in public places, such as a restaurant or theater. When entering a hospital or other medical facility becomes necessary, medical practitioners will have immediate and accurate knowledge about your condition, medications, emergency contact and other related information.
There are two ways to obtain a card. You can insert your personal information and then print a copy on this page, or you can send us your mailing address and we will send you a card to complete and a clear plastic holder attached to an attractive lanyard that can be worn. If requested, we will be happy to include a copy of ALS Worldwide’s booklet “A Paradigm Shift in ALS/MND Clinic Care: Best Practices from the Patient Perspective,” and a DVD of Ben Byer’s award-winning film about his and others’ odyssey through ALS, “Indestructible.”

These exercises are easy to follow with the aid of a caregiver or friend.
